Mega-dose Intravenous Steroid Therapy in the Sudden Deafness

Abstract
Background and Objectives: The purpose of this study is to compare the efficacy of mega-dose intravenous steroid injection with oral steroid medication in the management of idiopathic sudden sensorineural hearing loss (ISSHL).

Materials and Methods: 101 patients treated for ISSHL were evaluated between February 1999 and December 2002 retrospectively. After Routine physical examination, audiometric test including pure tone audiomery was done before and during treatemnt, at follow up visit after discharge. Mega-dose steroid iv group were given hydrocortisone 1,000 mg and 500 mg for first 2days in 48 persons, Oral steroid group were given prednisolone 80 mg for 4days and tapered for 10days in 53 persons. Steroid iv group was on same protocol as oral steroid group except intravenous injection for first 2days.

Results: Hearing recovery rate more than 15 dB was 77.1% in mega-dose iv group and 64.2% in oral steroid group (p>0.05). Recovery rate was significantly related to onset of hearing loss, severity of hearing loss, tinnitus, the duration of admission. but vertigo, age, sex had not significance.

Conclusion: Initial mega-dose hydrocortione injection had no advantage over prednisolone oral medication in management of ISSHL.
